Application for Admission
Below are the steps necessary to apply for admission to Holy Angels Academy. Beginning December 2021, all applications may be submitted electronically using the link below. One application can be submitted for your entire family. A $25 application fee and a $3 convenience fee applies for the family.
If you prefer to submit your application on paper, we need one application per child. The application is found at the end of the information packets below. A $25 application fee applies for the family.
For returning families, it is not necessary to schedule a visit or submit any records we already have on file. It is necessary, however, to submit an application for the upcoming school year.
1. Review Information Packet
A thorough review of the material contained in the information packet is the first step to help parents determine if they desire to enroll their children at Holy Angels. This includes information on our program, schedule, tuition, and more.
2. Tour Campus
It is strongly suggested that parents tour our campus before submitting an application. A visit may be arranged by scheduling an appointment with us.
3. Complete Application and Required Documents
The following are required (if applicable) before your child(ren) can be enrolled. This can be submitted while completing your application, at a later date by updating your submitted application, or to the office directly (visits@holyangelslouisville.com).
- Scores on previous report cards and standardized tests
- Evidence of satisfactory behavior
- High school placement test scores for incoming freshmen
- Any test results related to learning or behavioral disabilities
- Copy of official birth certificate
- Copy of official baptismal certificate (if baptized)
- Any report cards are to be submitted and student work samples are normally required.
4. Submit Records
All medical records (i.e., immunizations, etc.) and any other required documents must be submitted by deadlines established by the Academy for enrollment to be officially in force.
Holy Angels Academy must abide by the State of Kentucky standards in certain regards.
Should any parents file for medical exemptions, it is incumbent upon them to provide approved documentation to the Academy.
5. Submit Registration and Textbook/Supplies Fee
Once you’ve completed the forms you downloaded above, one for each child, please send us the completed forms and any records you wish to include.
You are welcome to mail these to us at the address below, bring them to the school during school hours, or send them electronically by clicking below.
If submitting electronically, payment for fees should be sent to the school via mail or dropped off during school hours.
All fees must be submitted by the designated deadline (listed in packet downloaded above) for enrollment to be in force.
6. Complete Interview
Should the application and accompanying documents indicate that a student is eligible to pursue enrollment, then the family will be notified to arrange an interview.
The interview session gives parents an opportunity to expound upon their reasons for desiring the enrollment of their children.
Additionally, a more in depth presentation is given regarding the religious education, character formation and classical curriculum.
7. Sign Statement of Parental Agreement
This is normally signed and submitted at the conclusion of the interview should the student(s) be accepted.
It must be submitted as an enrollment requirement.
